TERMS & CONDITIONS

Cancellation Policy
  • If you know that you are unable to keep an appointment we have booked, please call as far in advance as possible.

  • Appointments cancelled or missed without a 24 hour notice will be charged a fee of $50.00.

  • Cheques that are returned for “not sufficient funds” will be charged the amount of the cheque plus a $20.00 charge.

  • If I arrive at a location and a session is unable to take place for reasons beyond my control (i.e., if the client is sleeping, ill, or otherwise unable to participate in the therapy session), I reserve the right to charge for the entire session.

Confidentiality
  • My professional standards and ethics, in addition to the Personal Health Information Protection Act (PHIPA, November 2004), require me to keep all information you provide to and discuss with me in the strictest of confidence.  I am not permitted to release any information about you including your involvement with me as a client, without your informed, written consent.

  • I may audiotape or video-record some of our sessions only with your signed permission.  Videos and recordings may be used in developing goals and re-evaluating therapy performance.  They may also be used as an instructional method when watched again at home. I keep a written record of all of our contracts.  These notes are confidential documentations of our therapy goals, sessions, and discussions. Although they remain my property, you may view them at any time.

  • The only exception to confidentiality applies to most health professionals in the prossionals in the province.  At any time I may be audited by the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) as a practicing clinician.  They may want to view my files to ensure that I am providing services in keeping with best-practice requirements for the province and profession.
